VENDOR REGISTRATION

• NO ON-SITE REGISTRATION
• NO REFUNDS will be processed until 30 days after the district meeting, which must be indicated via email with original receipt.

Booths and tables will be assigned by Vendor coordinator. Vending booth space is to be a standard 10ft by 10ft space. One table will be provided with each booth purchase. 

If selling Omega Psi Phi Fraternity, Inc. paraphernalia you must provide the current license vendor identification number issued by Omega Psi Phi Fraternity, Inc. on this form. Failure to comply with each requirement will result in a denial of the privilege to sell at scheduled meetings and functions.
